Job Description

Locums - Seymour, Indiana, Addiction Medicine Opening Clinical Role:

    • Dates Needed:
      • 12/4
      • 12/5
      • 12/6
      • 12/9
      • 12/23 12/24
      • 12/26 12/27
      • 12/30 12/31
      • 1/2 1/3
    • Each of these days would be 6am - 12pm
    • Provide methadone or suboxone assisted treatments
    • Locum need to have a Suboxone x waiver but I think the State of Indiana has done away with that requirement. But if possible let's stay with yes to be safe
    • Number of Patients: Mainly the Intakes/Admissions then maybe a few random patients here and there. No more than 12
    • History and Physicals: The MD but we will try not to schedule on these days
    • Support Staff: Primary support will be Nurse Supervisor and her Team
    • EMR: Methasoft
