• Must have active state license
• BC required
• Days
• 8 hour days (can be flexible)
• 16 - 24 patients per day (mix of consults & procedures)
• Epidural, facet, transforaminal, medial branch & large joint such as shoulder or knee
• Credentialing not needed
• DEA needed
• Paid malpractice insurance; pre-paid travel and housing expenses
• Assignment details and time entry in online portal
