• Schedule: 2 days per week in the outpatient surgery center next door and 3 days per week in the clinic setting
• Job Setting: Outpatient/clinic
• Procedures: All epidurals, facet injections, ablations, and other procedures are preformed at the surgery center
• Credentialing: 0 days

• Board Certified/ Board Eligible: Board Certified or Board Eligible
• Certifications: ACLS, active Flouro is required
• Licensure: Active California
