Governed by community volunteers, we are a Federally Qualified Health Center with seventeen practices and program service sites across various locations. We offer fully integrated primary care services, covering family medicine, geriatrics, pediatrics, dental, mental health, speech and hearing, nutrition, substance use disorder treatment, podiatry and chiropractic medicine, primary care pharmacy, shelter, and healthcare for those experiencing homelessness. We empower individuals to advocate for their personal goals and needs, connecting them with community-based resources. Our success is measured by the overall health of our community, encompassing physical and emotional wellness, personal dignity, and a sense of belonging.

Requirements:

The position is planned for a full-time, 40 hour week

Clinic hours are generally of 8:00 a.m. ' 5:00 p.m.

Preferred license and certification as Physician Assistant or Certified Nurse Practitioner
