Rates: With Travel: $60/hr Without Travel: $65/hr Provider Qualifications License: Active Indiana State Medical License + active Indiana DEA (we can address change their DEA) Training: 1+ years experience Board Requirement: Board Certified Life Certificates: Active BLS and active DOT certificate Health Requirements: Credentialing Requirements: See Job Notes Tab Privileges: 1 week or less if they have all documents ready to go Other Notes: Job Details - refer to Job Notes for Presentation & Credentialing notes/timelines Duration: Ongoing Reason for coverage: LT needs Specialty: NPOM & PAOM Schedule/Hours: PRN ONLY, but clinic is open Monday- Friday 9a-6p Call: No call Census: 20-25 pt per day Setting: Clinic Duties: DOT & pre-employment physicals & on the job injuries Required procedures: Outpatient procedures Ages: EMR: Occuflex Travel allowed: Yes - look at Job notes for client's travel Other Notes: NP/PA Responsibilities: *Evaluate and treat patients in accordance with Nova Medical Centers medical model and standard of care. *Ensure compliance with licensing, certification, and accrediting boards *Perform physical, mental status, and medical history examinations; record findings subsequently reviewed and countersigned by supervising physician. *Follow patient progress and make changes in treatment as necessary *Participate in business development, when requested, to discuss health and safety needs and conduct workplace walk-through and other on-site assessments. *Make phone calls daily to client companies and communicate if patient needs to take off of work and length of time, etc. *Perform other tasks and duties reasonably requested by the Chief Medical Officer in accordance with Nova Medical Centers policies and procedures. Requirements: *Current State Physician Assistant or Nurse Practitioners Licensure * Current State DEA licensure * State Controlled Substance Licensure, if required by State Law * Current CPR Registration * Current Registration with National Registry of Certified Medical Examiners * Experience in UC, ER, FP, Occ Med, or Ortho, preferred * Ability to suture a laceration, diagnose orthopedic injuries, read ortho x-rays autonomously, and remove free-floating eye debris * Authorized to work in the USA * Ability to work in a cooperative, fast paced, multidiscipline setting * Excellent communication and computer skills * Must be able to efficiently enter data into Electronic Medical Records system * Good customer service, organizational, and time management skills * Good judgment, dependability and integrity * Professional appearance and demeanor * Spanish/English bilingual a PLUS but not required === INDY posting, edit only in INDY ===
