• Min 6-8 shifts per month
• 12 hour shifts; 6a, 7a, 10a, 1p, 5p, and 7p
• No call
• 60,000 annual visits
• Clean file required, no claims or gaps in timeline
• Active FL license and DEA
• EMR used is McKesson
