• Board Certified or Board Eligible in Family Medicine within 5 years.
• Active Idaho license required; BLS and DEA certifications necessary.

Credentialing Requirements:

• Quick credentialing process, approximately 7 days.

Patient Demographics:

• Ages 2 and up, with a focus on preventative health and physical exams.
• Average of 2 patients per hour.
