o Required experience, training, and certifications:

• Board Certified or Board Eligible within the last 3 years

o Case Logs:

• 50 patients (surgical or consultative) in the last 12 months

o Consider IMLC or waiting for a license:

• Will consider IMLC with LOQ

o Credentialing Timeframe & Certification Requirements:

• Typically 30 days, but applications are prioritized according to the start date
• Certification:
• BLS, ACLS

o Credentialing Timeframe:

• Covered by client and coordinated by Cuative
