- This is a permanent position
- Working in the town of Valley Stream on Long Island
- 15 -20 patients per day
- Full-time position
- Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
- Willing to train new candidates
- Experienced candidates may be eligible for increased compensation due to prior experience in a post-acute care setting

- Competitive compensation, with an estimated salary range of $161,000/annually to $201,000/annually with the opportunity to earn incentives.
