Compensation: $225,000 - $275,000, based on experience

You will participate as a member of the PACE Interdisciplinary Team in the care of high-risk older adults living in the community with a strong focus on the participant's goals as well as excellent geriatric care management.QUALIFICATIONS- At least one year of experience working with a geriatric population- Current MA license, DEA prescription privileges, and CPR CertificationPreferred - BC in Geriatric Medicine- Suboxone Waiver- At least one year of experience working with low-income and diverse cultural groupsAbility to- Quickly recognize situations/problems before becoming acute, implement effective solutions, and understand consequences- Convey information clearly and succinctly to team members
