• Must have active state license
• BC or BE required
• Weekdays, Days, No call
• ACLS and BLS needed
• 8 - 13 cases per provider
• General, ortho, OB, vascular, urology & must be able to do ultrasound guided regional blocks
• Credentialing needed
• DEA needed
