Long term coverage needed for two weeks a month.

Schedule and Setting:

2 weeks a month back-to-back:

-1st week is clinic Monday Friday 8a-5p with no call

-2nd week is 24-hour call Monday 7a Monday 7a

BLS and ACLS required

BC or BE in first 5 years

Rate caps have been removed.

*Required for privileges: 150 Urology Surgery Case Logs.

Board Certified/ Board Eligible

BLS & ACLS

General Urology Adult

Procedures required (see attached)

Support Staff: RNs: 4, MA: 10, Referrals:3, PSS: 4, Surgery Scheduler: 1, Financial Counselor: 1

REQUIRED:

WA state license; active, unrestricted or IMLC

Board certified Urology

COMPENSATION:

Competitive Market Rate (rate based on availability, experience, certifications, housing and transportation needs, etc.)

Malpractice insurance provided; all housing and travel covered
