Your responsibilities include:

• Enhance cognitive, psychosocial, and vocational interventions that empowers patients receiving physical rehabilitative care to achieve an optimal level of overall functioning
• Cultivate independence and personally fulfilling, socially-emotionally meaningful, and functional effective interactions
• Establish patient goals to facilitate independence, social, and psychological adjustment
• Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
• Make recommendations on transitional care or outpatient services after short stay
• Develop intervention plans, and share ideas about network management issue
• Facilitate and assist leadership in quality improvement initiatives and risk management
• Represent TeamHealth in engaging, establishing, and maintaining relationships within the community
• Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
• Timely response in chart completions
