Opportunity Highlights

• Part-time contractor work 10- hours/week
• Physician will be traveling to SNF's and assisted living facilities throughout Humboldt county (Eureka, California)
• See 10-20 patients/week
• Make your own schedule! Flexibilityto round at your own pace and make visits any reasonable time of day
• No on-call requirements
• Strong work-life balance, ongoing training, mileage reimbursement, and facilities which are very close in proximity. • Our processes are set for you to focus more on patient care rather than paperwork. You will function as a clinical leader by providing decision support to patients, families, and facilities. You will perform preventive, routine, and/or urgent care visits to optimize chronic conditions, assess safety of living conditions, educate patients and caregivers, and develop proactive care plans.
• You will coordinate with other clinical care providers across the continuum of care, including physicians, nurses, and therapists to ensure smoother transitions and reduce unnecessary hospitalization. You will also document visits in our EHR.
• Pay is per patient with base $140/hour (accumulated pay is based on provider's speed)
• Board Certification in Geriatrics is NOT required, Family Medicine and Internal Medicine physicians may apply
