• Board-Certified Nurse Practitioner
• NJ license/DEA in hand
• Seeking candidates with 1+ years of Home Health or Medicare populations
• NP's make 6-7 visits daily in a defined territory
• Will collaborate with Home Health, Specialists, Social Workers, Hospitals, SNF's, Etc.
• Support staff available
• Ages seen are mostly 55+
