FNP Opportunity at Outpatient Advanced Primary Care Clinic

We seek a skilled and compassionate Family Nurse Practitioner (FNP) to join a new advanced primary care health center in Liverpool, NY (10 minutes from Syracuse). This is a solo provider site, supported by one Medical Assistant (MA), and will serve a patient population of 300-500 individuals. The role involves managing a range of health conditions, with a focus on chronic disease management and some occupational medicine (including DOT physicals). Key Responsibilities:

• Provide primary care and chronic disease management
• Conduct DOT physicals and other occupational health services
• Work collaboratively with the Medical Assistant to ensure high-quality patient care
