DETAILS:- Critical member of the Street Medicine Program who will work in various environments, such as encampments, shelters, and transitional or permanent housing- Participate in community health initiatives, advocacy efforts, and outreach activities to foster trust and build lasting relationships within the community-Travel with the mobile unit to encampments, shelters, and community sites to bring care directly to patients- Collaborate with a multidisciplinary team, including social workers and community health workers- EMR: Epic- 2+ years of experience in community health REQUIRED- Bilingual (SPAN/ENG) REQUIRED
