- Develops individualized treatment plans based on psychiatric diagnosis, identified problems and treatment needs for each case. - Utilizes social work systems and psychological theories, principals and techniques in treatment plan development. - Works with highly complex cases in which individuals are severely impacted by moderate to severe psychiatric diagnoses to include factors such as history of trauma, co-occurring conditions and suicide. - Analyzes information collected to determine psychiatric diagnosis, treatment plan and/or advisability of counseling or referral to other specialists or institutions. - Provides case management services to effectively manage patients enrolled in behavioral intensive outpatient behavioral programs and prevent crises. - Performs a variety of quality management/quality assurance duties to meet the Joint Commission (TJC)requirements. - Performs other duties as assigned.
